What is Help at Home?

Help at Home by Montefiore offers the care you need, when and where you want it. Our Care Coordinators will help you choose from our wide range of services, delivered by our highly skilled and professional care specialists.

How does it Work?

Help at Home is available through government subsidised Home Care Package (HCP) and as private fee-for-service care. Whatever your budget or needs, we can help you put together an individually tailored program.

HOME CARE PACKAGES (HCP)

Older members of the community may be eligible for a Home Care Package (HCP), offering Federal Government subsidies for care and services that will enable you to live independently. To apply, you can self refer for an evaluation by an Aged Care Assessment Team (ACAT) at myagedcare.gov.au, get a referral from your GP, or our Help at Home team is happy to assist you with the referral process every step of the way. WHAT IF I DON’T HAVE A PACKAGE?

If you don’t have a HCP or are waiting for one, Help at Home also offers private, fee-for-service care. Call our hotline for a
verbal quote or to arrange a consultation.
